Informed Consent Form
It is sometimes necessary for a veterinary surgeon to treat Game Birds with "off label" prescription medicines.
These treatments are only prescribed following a veterinary consultation and advice. In these instances we are obliged to inform the client that the medicine is "off label" and request that they sign a consent form giving their permission for the medicine to be used.
